Assessment of Credentials - General Instructions for Applicants
A. Eligibility Requirements
Eligibility to sit CCMG examinations is determined by the Credentials Committee after review of the candidate's completed application and supporting documents. The following requirements and guidelines apply to candidates who received medical genetics training in CCMG, Royal College or ABMG-approved training centre. Admission to the examinations is granted for two examinations cycles.
- General requirements
The candidate must have successfully completed a CCMG - accredited training program in their chosen specialty (see Training Guidelines) by June 30 of the year of examination.
- Credit for previous training
Candidates wishing to have credit for previous training should submit their application early in their training or preferably when planning their training program. Individuals with extensive research experience might be exempted from the research portion of their training.
- Application for a second specialty examination
Applicants who wish to take a second subspecialty examination must submit a new application specific to the area for which certification is required. In some cases, credit might be given to a portion of the previous training (see above).
- Application for re-examination
If certification has not been achieved within two examination cycles, the candidate must submit a new application for assessment of credentials. The candidate might be required to do additional training before being permitted to repeat the examination process.
B. Documentation
Individuals who wish to have their credentials examined with respect to eligibility for examination must submit the following:
1. application form
2. curriculum vitae
3. documentation of completed and planned training
a) this must include details of the dates of rotations, name of supervisor, site, and outline of experience
b) and must be signed by members of the supervisory committee
4. logbooks of clinical/laboratory experience
a) a separate logbook for clinical and laboratory experience should be kept (see suggested format)
b) this must include date of patient visit/procedure, name of supervisor, site, and diagnosis (do not use patient name; identify case by laboratory/hospital ID number or other method which protects confidentiality)
c) cases recorded must document a broad based exposure
d) must be signed by members of the supervisory committee
e) the log books must be sent by e-mail to ccmg@rcpsc.edu with a cc to your Training Director. A hard copy letter from the Training Director attesting that he/she has seen the logbooks and signed off on them must be submitted to the CCMG office.
f) the log book should only be e-mailed once the case requirements are fully complete, or when an update is required by the Committee for credentials review after the originals have been first submitted for verification by your Training Director.
5. in-training evaluation forms
a) for each rotation
b) for long rotations, an evaluation must be done at least twice yearly
c) must be signed by the supervisor, and the program director
d) use the CCMG standard in-training evaluation forms
6. two letters of reference
a) sent under separate cover - may be issued as hard-copy or electronically
b) from individuals familiar with medical genetics, preferably members of the CCMG
C. Submitting Documentation
Applicants must submit a hard-copy of the entire application, including application fee, to the CCMG office at:

D. Fees
The application fee is $500.00 Canadian. For applicants in 2 specialties, the fee is $500 for the first specialty and $250 for the second.
1. a cheque or money order payable to the Canadian College of Medical Geneticists must accompany the application
2. fee is non-refundable
3. fee covers credentialing process only (examination fees are to be submitted to the Examination Committee after approval by the Credentials Committee)
E. Deadline
Deadline for submission of documentation and fee is August 1 of the year preceding the examination.
1. the credentials committee meets once yearly at the annual CCMG meeting and applications are reviewed at that time
2. individuals with incomplete files will not be considered; the candidate and his/her supervisory committee must ensure that documentation is completed in a timely fashion
3. examinations are held every two years
F. Final In-training Evaluation
Because credentialing might be done several months prior to completion of training, the committee must receive the standard final in-training evaluation form documenting completion of training prior to the oral examinations.
G. Exemptions
Fellows of the Royal College of Physicians and Surgeons of Canada in Medical Genetics Clinical Geneticists who have successfully passed the RCPS examinations in Medical Genetics are eligible for fellowship within the CCMG without examination. They can apply for credentialing by submitting one copy of their CV, proof of fellowship in the RCPSC, two letters of reference from CCMG Fellows, and the fee of $150.00. This applies for Clinical Genetics only. Individuals seeking credentialing in the other specialties must comply with the standard requirements.
