Verification of logbook entries:

The Training Director must review all entries in the trainee's logbooks, verify that the trainee had a major involvement in the cases listed and submit a signed letter to the CCMG electronically, attesting that he/she has reviewed the logbooks and signed off on them. The CCMG office must receive this letter by August 1 of the year that your credentialling application is submitted.

Logbooks:

There are specific templates for the "Cytogenetics Technical" and "Cytogenetics Interpretative and Consultative Skills" logbooks. Other specialties are to use the general templates. Cytogenetics trainees are to use the same Clinical template as the other specialties.

All Cytogenetics applicants are required to submit a completed "Summary sheet for Cytogenetics Logbooks".
