Specialty Logbooks & ITERS

Trainees must maintain an accurate logbook of their training and rotations.

The Training Director must review all entries in the trainee’s logbooks, verify that the trainee had a major involvement in the cases listed and submit a signed letter to the CCMG electronically, attesting that he/she has reviewed the logbooks and signed off on them. The CCMG office must receive this letter by August 1 of the year that your credentialling application is submitted.
